Clinical Practice Partner

Medical Surgical Units

Sherman Hospital

FT Days

Major Responsibilities:

  • Educator: Develops and provides ongoing educational resources for providers/clinicians and for patients to ensure availability of up-to-date information regarding patient care and maintenance/enhancement of clinical skills.
    • 1)Implements/evaluates budgets related to department education.
    • 2)Coordinates and facilitates orientation plan(s) as appropriate to roles and job descriptions and experience of newly hired staff.
    • 3)Mentors and consults with developing nurses and other clinicians regarding their needs.
    • 4)Prepares and presents formal and informal in-services for nursing staff/ancillary staff/medical staff/ residents.
    • 5)Provides staff with current clinical information.
    • 6)Assesses, coordinates, implements and evaluates programs for staff development including facilitation and program management.
    • 7)Collaborates with managers and directors to develop clinical excellence programs and measure competency.
    • 8)Completes required contact hours based on FTE status, within time frame.
    • 9)Develops curriculum and actively presents educational sessions based on unit needs assessment conducted in partnership with departmental leaders
    • 10)Promotes specialty certifications within department and provides educational resources as appropriate.
  • Practitioner: Demonstrates expert clinical judgment and autonomy, provides providers with recommended guidelines to achieve optimal levels of practice and outcomes.
    • 1)Performs nursing interventions to provide care to patients and their support systems with a high degree of skill and knowledge.
    • 2)Initiates or facilitates interdisciplinary dialogue to establish a comprehensive and consistent plan of care for patients.
    • 3)Consults with physicians and surgeons in the development and evaluation of care plans for patients and for support systems with complex needs.
    • 4)Role models the nursing process to improve practices.
    • 5)Promotes patient safety event reporting and participating as requested in departmental and hospital wide safety initiatives.
    • 6)Consults with leaders to ensure through put and access to services provided throughout the division. (i.e. preoperative services, scheduling, and day of surgery experience for patients and physicians
  • Administrative/Leadership: Demonstrates leadership qualities that reflect accountability and interdependence consistent with goals of the organization.
    • 7)Reviews and recommends changes in departmental policies, revises procedures in conjunction with new or changing theories, standards of practice and current technology.
    • 8)Participates in and supports Performance Improvement/Clinical Excellence initiatives within the Medical Center and system wide.
    • 9)Coordinates special projects as assigned.
    • 10)Encourages staff involvement in professional organizations and activities.
    • 1)Partners with team leaders, ACMs, manager and directors to provide leadership to accomplish nursing strategic and departmental KRA goals.
    • 2)Participates in the development, implementation and evaluation of department budgets as appropriate to role/accountabilities.
    • 3)Participates in problem identification and resolution with appropriate unit staff and leadership.
    • 4)Provides clinical expertise to budget related decisions.
    • 5)Facilitates the development and review of policies and procedures to ensure quality nursing care.
    • 6)Assures that standards of care are being practiced in accordance with regulatory agencies, i.e., AORN. Accrediting Organization, IDPH, AOS, etc.
  • Consultant: Serves as a consultant and/or resource person for medical center personnel in the continuum of services. Collaborates with physicians, nurses and other members of the interdisciplinary team in planning the care of patients needing specialized nursing and care coordination.
    • 1)Mentors individual staff members in their clinical development.
    • 2)Provides clinical resources for nursing staff and healthcare providers.
    • 3)Provides consultation to staff, management, administration and other disciplines related to departmental policies and protocols and AORN clinical guidelines.
    • 4)Provides peer consultation and support to team members as appropriate.
    • 5)Disseminates professional/clinical knowledge to the nursing community through professional literature, publications or public speaking.
    • 6)Participates in shared decision-making activities and committees.
    • 7)Provides related education to other departments as requested
  • Researcher: Promotes research findings by making appropriate recommendations for changes in policies, procedures, and practice.
    • 1)Integrates relevant nursing research into present practice.
    • 2)Assists staff in the application of research findings to professional practice.
    • 3)Role models research utilization.
    • 4)Expands the scientific base of nursing practice by participating in and/or conducting clinical research.
    • 5)Utilizes applicable resources and information systems to obtain and report outcome data.
    • 6)Participates in product review as requested.
    • 7)Facilitates unit based journal clubs.
    • 8)Participates in education council activities as assigned.


Licensure:

  • Nurse, Registered (RN)


Education/Experience Required:

  • * Bachelor's prepared (BSN, BS, etc.) required. * 3+ years recent experience in clinical specialty. * Graduate from an accredited school of nursing. * Enrolled in a masters nursing program. This position will need to recognize the needs and behaviors of a variety of age groups of patients, i.e., understand normal patterns of adult behavior and look for signs of normal aging; prior experience with various age groups is preferred.


Knowledge, Skills & Abilities Required:

  • * Strong analytical skills. * Proven leadership skills. * Good organizational skills * Basic computer knowledge. * Effective communication and presentation skills.
  • * Current RN license in the State of Illinois. * Certification in specialty area, e.g. CNOR * BLS Certification.


Physical Requirements and Working Conditions:

  • * Ability to work flexible hours according to customer needs. * May be exposed to hazardous materials and life-threatening diseases. * Ability to prioritize independently and respond to multiple simultaneous requests. * Lifting up to 30 pounds occasionally and/or up to 10 pounds frequently. * Ability to communicate with patients/families. * Ability to travel locally. * Ability to work under stressful conditions and in difficult situations.
